How do I take/use this medicine?

Take Clomipramine exactly as directed by your doctor. Do not take more or less than instructed by your doctor.

You may need to take Clomipramine for some time before the full benefits can be felt. Do not be discouraged if you do not feel better soon after taking the medicine. Clomipramine must be taken regularly for it to work well.

Do not stop taking Clomipramine unless instructed by your doctor. You may feel unwell if Clomipramine is stopped suddenly. When your doctor decides that you do not need Clomipramine anymore, he will usually reduce your dose slowly. Follow the doctor's instructions carefully.

You may take Clomipramine with or without food.

Clomipramine is available as conventional tablets or capsules. It may also be available as a sustained-release form. Sustained-release forms are "slow-release" because they release medication slowly over a period of time. These forms are usually marked as "SR", "XL" or "XR".

If you have been given the sustained-release type of Clomipramine, swallow it whole with a glass of water. Do not chew, crush, cut or open it.

Always keep your appointments with your doctor so that he will be able to monitor your response to Clomipramine.

What should I take note of while using/taking this medicine?

Inform your doctor if you have any of these medical conditions:

- schizophrenia or other psychiatric problems
- a history of epilepsy (fits)
- past or recent brain injury
- thyroid disease
- heart, liver or kidney disease
- low blood pressure
- frequent constipation
- an adrenal gland tumour such as phaeochromocytoma or neuroblastoma
- Parkinson’s disease
- a blood disorder known as porphyria

Let your doctor know if you are undergoing electroconvulsive therapy (ECT).

Call your doctor immediately if you notice any of these signs:
- worsening of your depression with thoughts of harming yourself or others
- if you have made any attempts to harm yourself or others

Alert your doctor if there are any significant changes in your behaviour or mental state.

Clomipramine increases the risk of suicidal thinking and behaviour in children, teenagers and young adults (24 years old and younger). However, depression and some mood disorders are themselves associated with an increased risk of suicide. It is important for you and your family members to monitor your condition while you are being treated.

You may need to have regular blood tests to monitor your body's response to Clomipramine. These blood tests will be more frequent in the first few months.

If you are going for an operation, including minor operations and dental work, inform your doctor or dentist that you are being treated with Clomipramine.

When should I not use this medicine?

Alert your doctor if you have any kind of heart disease or a recent heart attack.

Alert your doctor if you have bipolar disorder, mania, glaucoma (high pressure in the eye) or difficulty passing urine.

It is also important to inform your doctor if you are pregnant or if you become pregnant while being treated with Clomipramine.

Do not breastfeed while being treated with Clomipramine.

Do not take Clomipramine if you are taking or have taken other mood medicines known as monoamine oxidase inhibitors (MAOIs) in the last 14 days. Some examples of MAOIs include isocarboxazid, phenelzine, moclobemide, selegiline and tranylcypromine. Alert your doctor immediately if you are taking or have taken any of these medicines or other MAOIs.

Are there any restrictions on the type of food I can take?

Avoid alcohol.

Why do I need this medicine?

Clomipramine is used to treat depression.

It can also be used to treat obsessive-compulsive disorder (OCD), panic disorders, phobias and post-traumatic stress disorder.

Clomipramine can also be used to treat other conditions such as certain sleep disorders, nerve-related pain and adult bed-wetting.

What side effects could I experience?

Clomipramine may make you drowsy or dizzy. If you feel drowsy or dizzy, do not drive or take part in any activity in which you need to be alert. Dizziness may be worse especially when you get up from a sitting or lying down position. To minimise this problem, sit or stand up slowly.

You may also have some problems with your vision, such as blurred vision. Always inform your doctor of any changes in your vision. Avoid driving and operating machinery if you are not able to see clearly.

Other common side effects of Clomipramine include:
- dry mouth
- constipation or diarrhoea
- nausea, vomiting
- stomach discomfort
- appetite increase or loss
- weight gain or loss
- weakness or tiredness
- rash
- hair loss
- increased sweating
- hot flushes
- changes in sex drive
- headache
- difficulty sleeping
- nightmares
- ringing in the ears

Inform your doctor if these symptoms are severe or refuse to go away.

Your skin may also become more sensitive to sunlight. Protect your skin from the sun with sunblock, an umbrella or a hat.

Other side effects are less common but may need immediate medical help. Alert your doctor immediately if you develop any of these symptoms:
- irregular or fast heartbeat
- yellowing of the skin and white of the eyes (jaundice)
- breast enlargement in men and women
- inability to pass urine
- tingling and numbness in the hands and feet
- muscle stiffness, difficulty in controlling movements such as trembling or shaking of hands and feet or shuffling of feet when walking
- severe sore throat and fever
- unusual bleeding or bruising
- loss of mental alertness
- confusion or hallucination
- fits or seizures
- increased irritability
- difficulty in speaking

Alert your doctor if you experience any other side effects while being treated with Clomipramine.

How should I store this medicine?

Store in a cool, dry place, away from the reach of children.

Medicines must not be used past the expiry date.

Can I take/use this with other medicines?

Do not take Clomipramine if you are taking or have taken other mood medicines known as monoamine oxidase inhibitors (MAOIs) in the last 14 days. Some examples of MAOIs include isocarboxazid, phenelzine, moclobemide, selegiline and tranylcypromine. Alert your doctor immediately if you are taking or have taken any of these medicines or other MAOIs.

Alert your doctor if you are taking any of these medicines:
- any other antidepressants
- blood pressure medicines such as clonidine, guanethidine, debrisoquine and bethanidine
- selegiline (a medicine used to treat Parkinson’s disease)

You will also need to inform your doctor if you are taking any of these:
- any other medicines used to treat depression and other mood disorders, especially lithium, fluoxetine or alprazolam
- epilepsy (fits) medicines
- hay fever, allergy or travel sickness medicines
- cimetidine (a gastric medicine)
- heart medicines such as digoxin, propafenone, flecainide, and quinidine
- disulfiram (a medicine used to treat alcohol addiction)
- blood-thinners such as warfarin
- methylphenidate (a medicine used to treat attention deficit hyperactive disorder)
- birth control pills

Always inform your doctor and pharmacist if you are taking any other medicines, including herbal tonics, supplements and medicines that you buy without a prescription.
